New to the Health Center? Parent Steps to Take:

  1. Complete Parent Permission Form (to be filled out once annually)

  2. Request an Appointment for:

    1. Behavioral/Mental Health

    2. Medical Services

  3. If requesting a Sports Physical:

    1. Complete Permission Form

    2. Complete Sports Physical Form

    3. Request a Medical Services Appointment - Don't forget this step

Sports Physicals

Best practice would be for your child's Primary Care Provider to conduct the sports physical as they are the most familiar with the health of your child. When your child has their annual well-check, you can request the provider to complete the sports physical form at that visit. Doing this saves you time, and it is helpful if your child should choose to play sports. The Health Center provides limited sports physical appointments.

Providence Express Care and Urgent Care offer Sports Physicals for a fee of $40-50. You can visit their websites and reserve an appointment. They often have same-day appointments available.

To request a sports physical appointment make sure you request a medical appointment after you fill out the Sports Physical Form below. An appointment for a sports physcial will not be made without requesting the medical appointment.

Students currently enrolled in a school with a Health Center clinic are eligible for medical and behavioral health care. We follow Washington State guidelines with regard to parental permissions and providing medical and behavioral health care.
