New to the Health Center? Parent Steps to Take:

  1. Complete Parent Permission Form (to be filled out once each academic school year)

  2. Request an Appointment for:

    1. Mental Health Therapy

    2. Medical Services

  3. If requesting a Sports Physical:

    1. Complete Permission Form

    2. Complete Sports Physical Form

    3. Request a Medical Services Appointment - Don't forget this step

Sports Physicals

Best practice would be for your child's Primary Care Provider to conduct the sports physical, as they are most familiar with your child's health. When your child has their annual well-check, you can request the provider to complete the sports physical form at that visit. Doing this saves you time, and it is helpful if your child chooses to play sports. The Health Center provides limited sports physical appointments.

Best Med Urgent Care, Providence Express Care, and Providence Urgent Care offer Sports Physicals for a fee of $30-50. You can visit their websites and reserve an appointment. They often have same-day appointments available.

Students currently enrolled in a school with a Health Center clinic are eligible for medical care and mental health therapy. We follow Washington State guidelines with regard to parental permissions and providing medical and mental health care.
