Sports Physicals

Best practice would be for your child's Primary Care Provider to conduct the sports physical as they are the most familiar with the health of your child. When your child has their annual well-check, you can request the provider to complete the sports physical form at that visit. Doing this saves you time, and it is helpful if your child should choose to play sports. The Health Center provides limited sports physical appointments.

Providence Express Care and Urgent Care offer Sports Physicals for a fee of $40-50. You can visit their websites and reserve an appointment. They often have same-day appointments available.

Students currently enrolled in a school with a Health Center clinic are eligible for medical care and mental health therapy. We follow Washington State guidelines with regard to parental permissions and providing medical and mental health care.
