*It is advisable to consult the child's Primary Care Provider for the best course of action. If the child has already undergone a yearly wellness check with their PCP, they can call and request a sports physical form to be completed by their PCP. However, it is important to note that the form must be filled out within the same year as the wellness appointment.
The Health Center does provide limited sports physicals throughout the year, depending on provider availability. Contact The Health Center where your child attends school for more information (Garrison Middle School, Pioneer Middle School, Walla Walla High School, Lincoln High School).
