*It is advisable to consult the child's Primary Care Provider for the best course of action. If the child has already undergone a yearly wellness health check with their PCP, they can call and request for a sports physical form to be completed by their PCP. However, it is important to note that the form must be filled out within the same year as the wellness appointment.
